About the Palliative Care Resource Center

The Palliative Care Resource Center contains valuable resources and information for patients, family members and caregivers who are dealing with serious, life-limiting illness. The information is organized in five categories: Improving Quality of Life, Advance Care Planning, The Caregiver Experience, Payment and Insurance, and Stories.  Our intent is to provide education through the use of videos, real life patient stories, articles, and other media and to encourage you to have conversations with your loved ones and healthcare providers about the kind of care you want. It is important to us that you are empowered to be co-creators of your healthcare experience.

About Four Seasons Compassion for Life

Four Seasons Compassion for Life, a hospice, palliative care and research organization in Western North Carolina, is a leader in state, national and international efforts to set benchmarks for palliative care.  As a leading hospice and palliative care provider, Four Seasons has been ranked in the top percentile in the country for client satisfaction.  The organization has a 19 bed residential hospice facility, but the primary provision of both palliative and hospice care is in homes, assisted living facilities, and skilled nursing facilities throughout Western North Carolina.   

In September 2014, Four Seasons was awarded a grant extending through a  3 year timeframe by CMMI (Centers for Medicare and Medicaid Innovations) to study and develop a community-based palliative care model. The goal of this model is to improve access, healthcare outcomes, and patient/family satisfaction while containing costs. The provision of this website is one of several ways we have utilized grant funding to achieve these goals.
